Description: This class is a wonderful session for brand new providers as well as anyone in your HCS program responsible for coordinating and/or monitoring services, and for those involved in revising and renewing the individual’s IPC (individual plan of care) i.e. Program Manager, Case Manager/Care Coordinator, and Nurses. Essentially, this training is appropriate for anyone who will be part of the team that assists with planning and implementing services for the individuals in the HCS programs. It is also a great training for QA staff and those involved in billing or data entry, who may be entering information from the IPC into the Texas Medicaid Health Portal (TMHP).
Topics:
-We will review the 3608 (IPC) form from HHSC, discuss roles and responsibilities of the LIDDA vs. the Provider concerning the IPC at enrollment, transfer, renewal, and any revision.
-We will look at sample IPC’s and review each component of the form to ensure that you understand how to complete the form.
-We will discuss when and why revisions are done and procedures to follow when a revision or renewal needs to occur.
-We will also touch on some important things to remember and tips when entering renewals or revisions into TMHP.
So please contact us as soon as possible if you are interested in having any of your staff attend. Whether you are brand new to the program, need a refresher, or have just never received much-needed training, we hope this course will be extremely helpful to providers. We will provide certificates of completion for those who register.
