This will be on Day 2 of Twogether Consulting’s Care Coordination workshop “Admission, Post Admission & Important Information For Preparing For Survey” (8/25/23) from noon-1pm. For those attending the workshop, you are welcome to stay and eat lunch with us and check out the presentation.
